Enrolling is simple. You can enroll online with a credit card or through the mail by sending the enrollment form with a personal check or money order.
Each of these options requires that you complete a simple form and submit it with payment for the annual (non-refundable) enrollment fee. Payment will be accepted in the form of a credit card if you apply online, or if you are applying by mail, you may choose to pay with a personal check or money order.
To join this program and begin realizing savings on your prescription drug costs, you pay an annual (non-refundable) enrollment fee as follows:
> $25 per person ($20 if enrolling online)
> $75 per family of four or more ($70 if enrolling online)
