There are four simple ways to enroll. Each option requires payment at the time of enrollment. Please have your credit card information available when you enroll. If you enroll by mail, you have the option of paying with a personal check. Note:  You are not required to enroll your spouse.

The BadgerRx Gold enrollment kit contains an enrollment form and postage-paid return envelope for your mailing convenience. There are several easy ways to obtain an enrollment kit. You can also download a PDF file of the enrollment form at this website. Applying by mail is the only option that permits you to choose how you would like to pay the enrollment fee — by check or credit card.
